There have been three instances of near new Flashflood 500 x 70mm failure at the coupling under normal working pressure. The most recent was at a structure
fire at Otahuhu. Fortunately no one was injured. Hose failure is a serious safety issue for crews on an incident ground when there is an immediate
loss of water.
The NZPFU immediately notified FENZ of the incident and as a result of initial investigations the decision was made to immediately remove all Flashflood 500 branded 45mm and 70mm hose from service. Please refer to the attached FENZ National Safety Notice for the instructions which includes the identification details and photographs. The hoses are distinct and should be easily identifiable. However, if there is any doubt of identification of the correct hose, remove the hose from service and seek advice from FENZ.
FENZ are working with the suppliers to determine the possible cause of the failures and any necessary action regarding these specific hoses.
The removal of these hoses remains in place until further notice.
