All equipment failures must be logged into FENZ systems

Further to the safety alert withdrawing all Flashflood 500 hose from service, there have been other instances of other internal ferruled 70mm hose separating from the male coupling.

It is critical these and indeed all equipment failures are reported, otherwise it is difficult to get a timely response to a potential issue and subsequent hazard.

Please ensure these are logged in the first instance on the safe@work system so any trends can be identified early.

While it may appear a minor equipment failure at station level, it could be an issue reoccurring at other stations or with other equipment. If all failures are reported then the full picture can be understood and actioned accordingly. Early intervention could prevent later injuries.

Please contact Greg Lee at greg@nzpfu.org.nz if you are aware of any hose separations at your station or elsewhere. At this stage it appears to be one batch of hose coupled by an external provider, the NZPFU are working with FENZ to assess the situation with some urgency.

Members will be kept informed of any updates.

In unity,
National Secretary
Wattie Watson
